May I search for a form by the form title if I don't have the form number?
Search engines allow searching for a form by number, title, category or range.
Will all STD. forms be added to Forms Management Center's Website as Fill & Print eForms?
Only forms meeting certain criteria are placed as Fill & Print eForms on FMC's Website. Pending forms show a note indicating a target date. (Notes are displayed in red and adjacent to form titles.)
When is STD. Forms Price Catalog going to be available?
Paper Standard Forms Price List may be found under the Paper Standard Forms Area. Also, pricing information is included in forms search results.
Does the FMC plan to offer eForms formatted for MS Word?
FMC is considering developing eForms formatted for Microsoft Word.
Can STD. eForms be downloaded?
FMC is considering eBusiness options that will enable websurfers to conduct online transactions.
Is there a cost for downloading STD. eForms? If so, how will billing and payment be handled?
Because downloading of STD. eForms is a future eBusiness option, billing issues such as validating online transactions are being developed.
Why don't you offer STD. 678 as an "Automated eForm?"
STD. 678 is listed as a link to State Personnel Board's Website. SPB offers free usage of STD. 678 from its Website. eFormats include Acrobat. For your convenience, the following is the hyperlink to SPB: http://www.spb.ca.gov/index.htm. Select the State Application Form link.
Is there any way to hide the left-hand options panel of a PDF form so the entire form can fill the whole screen?
The default preview for all STD. eForms should show without the left-hand options panel. Please identify the form number and report the problem to eAssistance@dgs.ca.gov for resolution. If you are experiencing eyestrain as a result of this preview, please enlarge the magnification to a size that enables you to see things more comfortably.
How can I prevent blank pages from printing when using Fill & Print eForms?
Two things to do:
- Ensure that you're using Acrobat Reader 7.0 when using Fill & Print eForms on FMC's Website, and
- Please contact your IT professional to determine if your printer driver is current or has not become corrupted.
How can I prevent typed information from disappearing from a form field when tabbing to the next form field on a Fill & Print eForm?
The problem appears to be the result of using an outdated version of Adobe Reader. Acrobat Reader 7.0 or higher is needed in order to use Fill & Print eForms on FMC's Website. You may download the current version of Reader from Adobe's Website. A link to Adobe is on FMC's Fill & Print Webpage.
Where can I get a copy of a STD. eForm that I can save on my PC?
Standard State eForms may be purchased from the Forms Management Center. For more information, contact eAssistance@dgs.ca.gov.
Is it possible to save a version of a Fill & Print eForm with recurring information filled out?
End-users of Adobe Acrobat Reader are unable to save filled-out Fill & Print eForms. However, FMC offers Automated eForms that have connected databases. These eForms allow end-users to save information. For more information, eMail your mailing information, including fax and phone numbers to eAssistance@dgs.ca.gov. A marketing kit will be mailed to you within one week. Pricing information and an order form are included.
STD. 634 does not allow entry of some of the required fields, such as the letter symbols that must accompany entries in line 7B through 71.
FMC appreciates reports of anomalies such as you described with Fill & Print eForms. Please continue to report these types of problems to eAssistance@dgs.ca.gov so a resolution can be made.
Is there a way to change the font on STD. Fill & Print eForms? What I'm trying to do is put the Business Services Officer's name in the Authorize Signature block and then change the font to Signature style.
Font choices are predetermined (restricted) in order to preserve the integrity of Standard State forms.
When printing a Fill & Print eForm all of the non-editable text fields print with no spaces between sentences. Everything is jumbled together. I've checked and I do have Adobe Reader 7.0. Please advise.
What you describe could be the result of your printer driver becoming corrupted or outdated or is caused by an anomaly unique to your computing environment. Please try printing another eForm. If you experience the same results, it is likely that your computing environment is causing this printing anomaly. I've encountered printing problems when using Fill & Print eForms. For instance, bordered lines appear on printed pages or letters on the left side margin are cut off. How can I fix?
This may be the result of printing to a non-postscript printer or unique to your computing environment. Try printing to a postscript printer. eAssistance did not encounter this anomaly when printing to a postscript printer. If an end-user has JetForm Form Flow Filler installed on a PC, is downloading of Automated forms available from your website?
End-users must purchase Automated eForms from the Forms Management Center. Contact eAssistance@dgs.ca.gov for more information.
Is there a big difference between STD. 636A and STD 636?
Yes. Both Standard State forms are a "Report of Performance for Probationary Employee." The difference is STD. 636A is for "Performance for Budget Based Agencies." |
