Golden State Overnight (GSO) shipping charges are calculated using package weights, zip codes and fuel surcharges. Use the following documents to calculate shipping charges: STD. Forms Weights, and GSO State Rates (Use the Next Day Ground PM option for shipping rates. Zip codes can be found within the Zone Chart option. Fuel charges can be estimated via the Fuel Surcharge option). Prices, rates and weights subject to change without prior notification.
STD. 101 (REV. 9/2009), Appointment Calendar 2010, will be available to order after October 15, 2009. The corrected 2009/10 Attendance Summary STD. 640 (REV. 9/2008c) is available online only. STD. 640 (REV. 9/2008) is no longer available as it contains holidays that have been recently eliminated.
